Physicals are required for players prior to stepping on the field for 3rd - 6th grade full contact football and cheerleading.  Flag participants do not require a physical. 

Please bring copy of physical to equipment hand out at the beginning of season.

*Physicals will not be returned at the end of the year.  Please keep original and turn 
into middle school office if your player intends to play another sport for the school.

Physicals MUST be dated April 15, 2026 or later. Physicals dated prior to April 15, 2026 will NOT be accepted.
